Services are provided to impaired persons 18 years of age and older, persons of 60 years of age and older, and to their families when appropriate. The services provided include, but are not limited to : assisting in placement such as in an adult home, nursing homes, etc.; transportation when needed; companion services designed to assist a person who is unable to care for themselves and needs assistance with activities of daily living; and nursing home/personal care screenings performed by the Adult Services Worker for persons who receive Medicaid or will receive within a 6 month period. These services are intended to maximize self-sufficiency, prevent abuse/neglect/exploitation, and to prevent, delay, and/or reduce inappropriate institutionalization.

Provides case management, assessment and direct services to Virginia's citizens who have disabilities or who are over the age of 60. Specific services include home based services (companion, chore, homemaker services), adult foster care, adult day care, case management, nutrition, assistance for assisted living applicants and residents, and participation on nursing home preadmission screening teams.

It starts with a careful review of the situation and continues with an individualized plan of services. This plan includes options that make it easier to match the right services to the right needs and meet daily challenges - big and small. Also offers Medication Reminder systems and Personal Emergency Response monitoring. Accepts Medicare and Medicaid.

Funds companion services, for limited hours each week, to assist individuals (aged, blind, disabled) with personal care or light housekeeping tasks. Seeks to prevent institutionalization. Also, assessment for determining level of care when institutionalization is necessary.

Persons age 60 and older may benefit from a trained and supervised aide to help with light housekeeping tasks, grocery shopping and errand running. This service may average 3 hours per week and offer respite to a caregiver.

*Private Pay Staff Nurses perform the initial evaluation of a new client and develop a personalized care plan. *Companions includes tasks like meal preparation and assistance with walking. Will dedicate the time needed to help with lifes chores or play a game over a cup of tea. *Home Health Aides are the right choice for people who have a hard time managing the more basic activities of daily living. Assists with everything from personal hygiene to taking blood pressure and help feeding.

Provider of personal care, companionship, light housekeeping, transportation, errands, and laundry. Offers respite care. Agency is licensed by Virginia Department of Health. All caregivers are bonded, insured, and screened through human services and criminal background checks. Personal Care: Provides personal assistance, stand-by assistance, supervision or cues for individuals who are unable to do one or more of the following: eating, dressing, bathing, toileting, grooming, walking, or transferring in and out of bed/chair. Homemaker Services: Provides qualified seniors with a cleaning and companion service, which includes light housekeeping, meal preparation, grocery shopping, using the telephone, and managing money. This service may be provided for purpose of respite for family caregivers.

Adult Services are provided to impaired persons who are eighteen years of age and older and to persons sixty years or older. Adult services available include the provision of case management, home-based companion services, screening for Medicaid community-based care and nursing home placement, screening for admission to Adult Care Residents (Home for Adults), and other activities to aid the adult and their family.

The In-Home Services Program provides home care services to enable senior citizens who cannot perform basic tasks for themselves to remain in their own homes.

Provision of companion, homemaker, or chore services to elderly in need of in-home services.

Services include but are not limited to: Companion/sitters/safety supervision, light meal preparation, light housekeeping, transportation to physician and dental appointments, social events, etc. medication reminders, and care coordination. Accept clients on a 24/7 basis or a minimum of 3 hours per day.

Personal Care, Chore (help with heavy household tasks) and Respite Care.

Provided by trained home care workers who provide cooking, cleaning, shopping assistance, meal planning and laundry service. Services enable elderly people to care for themselves and to remain in a clean, safe, familiar environment for as long as possible.

Home and community-based services allow the elderly or adults with disabilities to remain in their homes rather than move to institutional placements. Screening for adult care residence and nursing facility placements is provided.

Provides personalized, in-home assistance for seniors, offering a comprehensive range of services including personal care (bathing, dressing), medication management, companionship, light housekeeping, meal preparation, transportation, errands, and more, allowing individuals to comfortably remain in their homes while maintaining independence and quality of life.

Services provided by Senior Companions include daily tasks such as companionship-talking and listening, transportation, peer- counselling, grocery shopping, meal preparation and light housekeeping.
