Areas Served: Counties of Bath and Rockbridge and cities of Buena Vista and Lexington. This program provides early intervention services to families with infants and toddlers, birth to age three, who have developmental delays or disabilities.

Case management functions as the door through which consumers must pass to access needed services. Provides assessment, linking and referral, and monitoring to people with intellectual disabilities within the three county areas. Serves both children and adults.

Area Served: City of Virginia Beach. Serves children from birth through age three who have developmental delays, atypical behavior, and/or a disabling condition that is likely to result in a delay. Children referred for services are reviewed by an eligibility team to determine their eligibility for services. If it is determined the child would benefit from services (i.e., is eligible) he/she will receive a full developmental assessment prior to beginning services. Services available to eligible children and families include, but are not limited to, service coordination; speech, physical, and/or occupational therapy; and educational services. Services are provided in the child's natural environment (e.g., the home, daycare, etc.) and are provided on a sliding fee scale. The program also offers center based classes and community based group activities for children.

Southwest Virginia Child Development Clinic offers a multi-disciplinary team approach for the evaluation of children and youth from birth to 21 years, suspected of having developmental delays and/or disabilities from our twelve county, three city area. The process involves testing, assessing, and making a diagnosis. We will make referrals and recommendations as needed to address the needs of our children and their families. Field clinics are conducted on a limited basis within the region. Diagnostic Evaluations are performed for the following: Attention-Deficit/Hyperactivity Disorder, Autism, Developmental delays, Mental Retardation, Behavioral/Emotional Disorders, Learning Problems, and other developmental disorders or disabilities.

Areas Served: Counties of Bland, Carroll, Grayson, Smyth, and Wythe, Cities of Galax, Marion. Purpose: Provides assistance to infants and toddlers (birth through 3 years) with special needs and their families. The children may have one or more of these conditions: a slow rate of development in language or motor skills, hearing or visual problems, and or a physical handicap.

Infant & Toddler Connection of the Heartland serves the following counties: Amelia, Buckingham, Charlotte, Cumberland, Lunenburg, Nottoway, and Prince Edward. The Infant & Toddler Connection of the Heartland provides supports and services in the child's natural environment (home, day care, etc) for children with developmental delays from birth to age three. Longwood University serves as the local lead agency for implementation of the Part C program for families who live in planning district 14. Off street parking is provided and program is totally accessible for the handicapped.

EPIC supports infants and toddlers, ages birth up to 3, with disabilities and/or developmental delays who live in Hampton or Newport News. A family-centered program that provides information and support to families of infants and toddlers between the ages of 0-3 with developmental delays. Free developmental screenings are provided for any child living in Hampton and Newport News under the age of 3. Staff help develop an Individualized Family Service Plan (IFSP), which includes the family's needs, preferences, and concerns regarding their child and their family. Each child/family is assigned to a Service Coordinator at the time of the referral/intake. The support coordinators then set up the tailored therapy sessions with the families to ensure the child reaches their full potential.

Provides an array of outpatient services for children with serious emotional disturbances, substance abuse disorders and intellectual and developmental disabilities. Services include psychiatric treatment, medication management, individual, family and group counseling, school-based day treatment, Intensive In-Home Services, Infant/Toddler Case Management, and case management for children 3-18. Services are for children only. Provided in each of the offices by clinical social workers, and counselors. Outpatient counseling services are brief in nature, often using "solution focused" or other brief models of behavior modification. Outpatient services are provided at four different locations: Main Center (adjacent to SWCC), Tazewell County Satellite Office (Tazewell), Russell County Satellite Office (Lebanon), and at the Buchanan County Satellite Office (Grundy). Services include: *Outpatient Mental Health Services for adults, children and adolescents; *Outpatient Substance Abuse Services for adults and adolescents; *Medication Services; *Crisis Services; *Mental Health Case Management for adults, children, and adolescents; *Mental Retardation Case Management for adults, children, and adolescents; *Substance Abuse Case Management for adults and adolescents; *Community-Based Prevention Services; *Infant Intervention Services.

Areas Served: Counties of Isle of Wight & Southampton; Cities of Suffolk & Franklin. Provides Early Intervention services for families, infants, and toddlers (birth to age three) who have been diagnosed as developmentally delayed or are suspected to be such.

Area Served: Chesterfield County. Provides evaluations at no charge to families. Provides therapy services and service coordination. This program serves children from birth to 3rd birthday with a diagnosed medical condition resulting in delays in development or atypical development and/or a delay in development.

Areas Served: Goochland, Powhatan Counties. This program provides intervention services for families, infants, and toddlers birth to age three who have diagnosed or suspected delays in development.

Areas Served: Alleghany County, Clifton Forge, Covington. The Infant and Toddler Connection of the Alleghany Highlands coordinates early intervention services for infants and toddlers, birth through age 3 with developmental delays, diagnosed conditions or atypical development. Services may include developmental screenings - eligibility determination - assessment - service coordination and other services to meet the child and family's needs.

This program provides evaluation and intervention services for families, infants, and toddlers birth through age three who have been diagnosed or are suspected of having a developmental delay. Provides early intervention services for families who have infant and toddlers (birth to age three) who have developmental delays, atypical development, or a diagnosed disabling condition in accordance with Part C of the Individuals with Disabilities Education Act (IDEA).

Area Served: Hanover County. This program provides intervention services for families, infants, and toddlers birth to age three who have diagnosed or suspected delays in development.

Part C Early Intervention provides a full range of services for infants and toddlers with developmental delays or disabilities and their families. Services aim to provide formal and informal support to meet the outcomes identified by families for their children. Provide outreach, coordinate services, and make referrals in the community as needed.

Provides programs to enable children with special needs, birth to age 21, to function at home, school, and in the community. The philosophy of care is to ensure that children with disabilities realize their maximum physical, emotional, social and educational potential in each of these environments. The CDRC team includes: * Developmental Pediatrics * Pediatric Orthopedics * Pediatric Dentistry * Psychological Services * Physical, Occupational, and Speech-Language Therapy * Prosthetics, Orthotics and Casting * Nutrition * Radiology * Social Services * Special Education - from infancy through high school Additionally, consulting or other specialty services are available in genetics, child psychiatry, pediatric neurology, pediatric anesthesiology, pediatric gastroenterology, endocrinology, otolaryngology, ophthalmology, urology, plastic surgery, physical medicine, and neurosurgery.

Provides a variety of programs and comprehensive support to those with intellectual disabilities. Services include residential and day support services through contractual agencies, respite care for persons with intellectual disabilities, and early intervention services for delayed and at risk infants 0-2 years (PACE).

Purpose: Coordinates early intervention services for children birth to 3 years old who are developmentally delayed and /or who have disabilities. Promotes and helps build a comprehensive family-centered early intervention system.

Area Served: Accomack, Northampton Counties. Provides early intervention services for infants and toddlers (birth through age three) who have developmental delay and their families.

Area Served: Counties of Caroline, King George, Spotsylvania, Stafford, and City of Fredericksburg. Provides early intervention to infants and toddlers with developmental delays from birth to 36 months; and to provide any help and assistance the family may need.

Specialty clinics available upon referral for children needing evaluation and follow up in the following areas: orthopedics, hearing impairment, eye imbalance, neurology, cardiology, diabetes, urology, pediatric surgery, hemophilia, cystic fibrosis. A sliding scale fee is applied, based on income.

System of supports and services designed to promote the greatest possible developmental outcomes for infants and toddlers with developmental delays and disabilities.
